Many scouts and adults have still not handed in their medical forms to Mr. Malko at 505 West Farms Road. Without a medical form, a scout or an adult will not be allowed into Ockanickon Scout Reservation. Please make sure that all medical forms have a copy of the insurance card (both sides) attached to the back. Additionally, make sure all information on the medical form has been filled out. Also, please make three copies of the medical form; one will go to the Ockanickon Health Staff, another will stay in the troop trailer, and the scout's family will keep the other. Please hand in medical forms ASAP to Mr. Malko if you have not already done so.
